Quality of life, cognition and bone metabolism in patiens with pituitary adenomas
In dieser Arbeit wurden 52 therapierte Hypophysenpatienten (Prolaktinome n =
19, hormoninaktive Adenome n = 8, Akromegalie n = 14, M. Cushing n = 11)
hinsichtlich LebensqualitÀt, Knochenstoffwechsel und Knochendichte sowie
kognitiver BeeintrĂ€chtigungen im Vergleich zu einer Kontrollgruppe mit 50 chronisch Kranken (Patienten mit SchilddrĂŒsenoperation) untersucht. Von besonderem Interesse waren die Form der kognitiven BeeintrĂ€chtigung sowie der Einfluss von Operation, Radiotherapie und Alter der Patienten zum Zeitpunkt der
Operation.
Laborchemisch wurden die Hormonparameter sowie Parameter des Knochenstoffwechsels bestimmt. LebensqualitÀt bzw. psychische und physische Symptome wurden mit dem Fragebogen QLS-H sowie der Symptomcheckliste SCL-
90-R erfragt. Zur Ermittlung der Knochendichte wurde eine QCT durchgefĂŒhrt.
Die kognitiven Leistungen wurden mit folgenden neuropsychologischen Tests
geprĂŒft: MWTB, VLMT, TMT A/B, Zahlennachsprech-Test, Zahlen-Symbol-Test.
Die statistische Auswertung erfolgte anhand des Wilcoxon-Tests fĂŒr nichtparametrische Stichproben und einer ANOVA bei Mehrfachvergleichen.
Hinsichtlich LebensqualitÀt und Knochendichte zeigten die Hypophysenpatienten
keine signifikanten Unterschiede im Vergleich mit den chronisch Kranken in
der Kontrollgruppe. Operation und Radiotherapie zeigten keinen Einfluss auf die
Knochendichte.
Die Hypophysenpatienten wiesen im Vergleich zu den Kontrollen signifikante
BeeintrÀchtigungen im Bereich von Aufmerksamkeit und Exekutivfunktionen
sowie im ArbeitsgedÀchtnis auf. Dabei schnitten operierte Hypophysenpatienten
in Tests zu Exekutivfunktionen und zum LangzeitgedÀchtnis signifikant schlechter
ab als nicht operierte Patienten. Die Radiotherapie zeigte keinen Einfluss auf
kognitive Leistungen.
Erstmals konnte in dieser Studie gezeigt werden, dass das Abschneiden in den
Tests zu den Exekutivfunktionen mit dem Alter der Patienten zum Zeitpunkt der
Operation korrelierte: bei Operation jĂŒngere Patienten zeigten signifikant
bessere Testergebnisse als Patienten, die zum Zeitpunkt der Operation bereits Àlter waren.
Hypophysenpatienten leiden oft auch nach erfolgreicher Therapie unter nachhaltigen kognitiven Dysfunktionen, fĂŒr die eine stattgehabte Operation aber
auch die TumorentitĂ€t als Ursache in Betracht gezogen werden mĂŒssen. Von
besonderer Bedeutung fĂŒr das Outcome nach der Operation hinsichtlich kognitiver
BeeintrÀchtigungen ist das Alter zum Zeitpunkt des Eingriffs.52 treated patients with a pituitary adenoma (prolactinoma n = 19, hormoneinactive adenoma n = 8, acromegaly n = 14, M. Cushing n = 11) have been examined regarding quality of life, bone metabolism and bone density as well as
cognitive impairment in comparison with a control group of 50 chronic invalids
(patients after surgery for the thyroid gland). The kind of cognitive impairment
as well as the influence of surgery, radiotherapy and age of the patients at the
time of surgery was the matter of particular interest.
The hormones and the parameters of bone metabolism were the measured
laboratory parameters. Quality of life and mental as well as physical symptoms
respectively were inquired by means of the questionnaire QLS-H and the Symptom-
Checklist (SCL-90-R). A quantitative computer tomography (QCT) was accomplished
to measure the bone density. The cognitive performance was examined
using the following neuropsychological tests: âMWTBâ, âVLMTâ, âTMT
A/Bâ, âZahlen-Nachsprechtestâ, âZahlen-Symbol-Testâ. The statistical analysis
was carried out by means of the Wilcoxon test for non parametric samples and
ANOVA for multiple comparisons.
The patients with pituitary adenoma didnÂŽt show significant differences in comparison to the control group regarding quality of life and bone density. Surgery and radiotherapy didnÂŽt have influence on bone density.
The patients with pituitary adenoma showed significant adverse effects in the
field of attention and executive functions as well as working memory compared
to the control group. Operated patients with pituitary adenoma performed significant worse in tests of executive functions and long-term memory than patients with pituitary adenoma without surgery. Radiotherapy didnÂŽt show an influence on cognitive performance.
This is the first study to show a correlation between performance in tests of executive function and age of the patients at the time of surgery. Patients with
younger age at the time of surgery showed significant better test results than
patients who were already older at the time of surgery.
Patients with pituitary adenoma often suffer from lasting cognitive dysfunctions
even after effective therapy. Surgery as well as the tumour entity have to be
considered as a possible cause for this fact. The age at the time of surgery is of particular importance to the outcome after surgery regarding cognitive dysfunctions
